Short answer: Yes, and the evidence is unusually consistent for a dietary pattern. A large meta-analysis found that adherence to the Mediterranean diet is associated with an 11 to 30 percent reduced risk of cognitive impairment, dementia, and Alzheimer’s disease, with the protective association holding up across dozens of studies and different populations.
What the Research Actually Shows
A comprehensive meta-analysis published in the journal GeroScience, available via Springer Nature, pooled data from studies published between 2000 and 2024 examining Mediterranean diet adherence and the risk of age-related cognitive disorders. The results were notably clear-cut: the combined hazard ratio for cognitive impairment among those adhering to the diet was 0.82, for dementia it was 0.89, and for Alzheimer’s disease specifically it was 0.70, meaning adherents had roughly a 30 percent lower risk of developing Alzheimer’s compared to non-adherents. The researchers concluded that the Mediterranean diet has strong potential as a non-pharmacological, lifestyle-based intervention for promoting healthy cognitive aging.
This finding isn’t isolated to one analysis. A long-term study from the InCHIANTI aging study in Italy, published in Nutrients and available via PMC, followed 832 participants for up to 18 years, with an average follow-up of just over a decade. Those with the highest adherence to the Mediterranean diet were significantly less likely to experience cognitive decline, and their annual rate of decline on a standard cognitive test was measurably slower than lower-adherence participants. A separate systematic review of 17 cohort studies, cited in a 2025 study published in Scientific Reports, found that people who followed a Mediterranean-style diet had a 23 to 35 percent reduced risk of developing Alzheimer’s specifically, a range strikingly close to the meta-analysis figures above.

The MedLey study, published in Nutrients, randomly assigned 137 healthy older adults to either follow a Mediterranean diet or continue their habitual eating pattern for six months, then assessed cognitive function using a comprehensive battery of tests. This kind of randomized design is considered stronger evidence than observational studies alone, since it can better isolate the diet’s effect from other lifestyle factors that tend to cluster together in health-conscious populations.
There’s also emerging evidence on a related eating pattern worth mentioning: the MIND diet, which combines elements of the Mediterranean diet with the DASH diet and places particular emphasis on foods believed to be especially protective for the brain. According to the National Institute on Aging, an NIA-funded study published in Neurology found that both the MIND and Mediterranean diets were associated with fewer signs of Alzheimer’s-related brain pathology at autopsy, with green leafy vegetables in particular linked to less disease-related damage in the brain. This is a genuinely valuable type of evidence, since it connects dietary patterns directly to physical, measurable changes in brain tissue rather than relying solely on cognitive test scores.
What the Diet Actually Involves
- High intake of vegetables, fruits, legumes, and whole grains. These form the foundation of the dietary pattern across nearly all the studies reviewed.
- Fish and olive oil as primary fat and protein sources. Most study definitions specify at least a few weekly servings of fish alongside olive oil as the main added fat.
- Limited red meat and moderate dairy. The pattern generally restricts red meat while allowing moderate amounts of dairy, distinguishing it from stricter plant-based diets.
- Moderate wine intake, where culturally appropriate. Some versions of the diet include moderate wine consumption, though this component is optional and not necessary to gain the diet’s core benefits.
Given the consistency across large meta-analyses, long-term cohort studies, randomized trials, and even brain pathology research, the Mediterranean diet stands out as one of the more solidly evidence-backed dietary patterns in the entire field of cognitive health research.
